Child Profile Health Promotion sends health promotion materials to all parents of children aged birth to six years in the state of Washington. The materials contain age-specific information about immunizations, growth, development, safety, nutrition, and other parenting issues.
There are a total of 18 mailings, timed to correspond with the American Academy of Pediatrics recommended schedule of well-child visits. Each mailing contains an age-appropriate letter and, depending on the age, may contain other useful materials that are listed by topic below. For the first 18 months, mailings are sent every three months. From 18 months to six years, mailings are sent every six months.
The Health Promotion materials consist of the following items:
A user does not need to be logged into the application in order to sign up for the free Health Promotion materials, but if logged in, the user should log out after completing and submitting the sign-up form.
To sign up for Health Promotion materials, locate the Valuable Links section on the home page and click the Child Profile Health Promotion materials link. The Health Promotions information page opens; click the SIGN UP link on this page (or click the link for the Spanish form). The sign-up form page opens.
At a minimum, enter information into at least the required fields. To add additional children to the form, click Add Another Child. Note an error message displays if the entered email address was registered previously. Click Submit when finished. A Thank You page appears.
To sign up additional family members to receive the Health Promotion materials, complete the sign-up form for each additional family member.
