Universal Certificate of Immunizations (Louisiana)
This generates a State of Louisiana University Certificate for Higher Learning form for a specific patient. You must first search for and select a patient.
This certificate is a modification of the Universal Certificate Forecast Settings specifically for Higher Learning with Tdap and DTaP separated.
Generate the Certificate
To generate this certificate, click the Reports > State Reports > Patient Specific > State of Louisiana Universal Certificate for Higher Learning link. (School Nurse users can also access this report after selecting a patient and clicking Reports > School Reports > School Nurse Reports > State of Louisiana Universal Certificate for Higher Learning.) The State of Louisiana University Certificate for Higher Learning opens in a new browser tab. Note any warnings on the page, such as This patient/child is past due and must receive vaccination(s) Printing this certificate with an Expiration Date prior to today's date may constitute fraud. To print a valid certificate, use the browser's print function.
